We use SimplePractice, a secure portal where we manage all scheduling, communication, and paperwork.
⚪️ Intake Paperwork
You’ll receive intake paperwork via email. All forms must be completed and signed before your first session.
⚪️ Credit Card on File
A valid credit card is required to confirm your appointment.
▹ Self-Pay Clients – Your card will only be charged after your session.
▹ Insurance Clients – A card is required for co-pays and any balance not covered by insurance.
▹ Lyra & Spring Health Clients – A card is required per practice policy (no charges will be made unless otherwise notified).
